How the Collaborative Works

The collaborative supports the development of model post-acute care clinics at leading academic medical centers. Each clinic brings together multidisciplinary teams to provide coordinated, evidence-based care for patients recovering from concussion and other traumatic brain injuries.

Participating clinics share clinical protocols, standardized assessments, and data to continuously improve care and outcomes across the network.

Core elements include:

  • Multidisciplinary clinical care — neurosurgeons, neurologists, physiatrists, psychologists, psychiatrists, and rehabilitation specialists

  • Full-time patient care coordinator

  • Weekly multidisciplinary care team conferences

  • Standardized patient symptom assessments

  • Proactive, coordinated follow-up after injury

  • Shared data and learning across institutions

  • Regular collaboration across participating sites
